Add standard SDLC documentation structure for the project:

ADR README with index of existing decisions
ADR template for new decisions
Scenarios directory for GWT specifications

Motivation

Support consistent documentation practices and make existing ADRs discoverable.

Scope

Documentation only - no code changes.
